Had about four crashes in beta-4 and two post-beta-3 SVN builds. (I don't remember precisely what I was running at the time of each,) but this crash log and saved game is a representative example.

What are the modifications to the source code? Your crash log originates from a customly modified version of OpenTTD.

Can you re-create the crash without using a custom-built version?

Lots of nested conditional orders. The smallstack used to leak items in that case which would eventually result in it being unable to allocate any further ones. I've fixed that today. Can you retry with something newer than r26343?
