Based on discussions on IRC, the # 1 source of confusion in OpenTTD is the fact that you can freely choose a start year, but there is no indication that you cannot actually do much before 1950 unless you have the proper NewGRFs installed.

This is a serious usability bug, because an action as simple as starting a new game leads to a situation that most people probably cannot resolve without searching the web or asking on IRC/forums.
